Recently, three types of gels have become the most commonly used: hyaluronic acid, polylactic acid, and synthetic liquid implants (Activegel), each with its own advantages and disadvantages.
These procedures are performed under local anesthesia: typically, an anesthetic gel is applied to the skin of the penis for 20-30 minutes, and a small amount of lidocaine solution is injected a few minutes before the gel injection.
Using hyaluronic acid or polylactic acid fillers for penile girth enhancement allows for up to 50-60 ml of gel to be injected, resulting in an increase in diameter by 1.5-2 cm or a circumference increase of 6-7 cm.
However, the procedure is typically performed in a series of injections of 10-20 ml, achieving the total volume of 50-60 ml over three sessions.
In England and Germany, hyaluronic acid gels are commonly used for penile girth enhancement, with up to 20 ml of gel evenly distributed along the sides and dorsal area of the penis.
The main disadvantages of hyaluronic acid gel are its mobility and tendency to shift: it often moves downward and accumulates near the glans, creating an unnatural pear-shaped appearance of the penis. It can also cause bulges and irregularities in other areas.
Additionally, it frequently migrates during vigorous sexual activity, leading to unevenness. When a large amount of hyaluronic acid gel is injected into uncircumcised men, the filler often accumulates in the foreskin, causing the glans to retract into it like a pouch, resulting in aesthetic and sexual dissatisfaction.
However, the most significant drawback of hyaluronic acid fillers is their relatively fast absorption rate: typically, such gel dissolves within 8 to 12 months, requiring repeated injections.

Much more physiological and rational for thickening and enlargement of the penis are injections of polylactic acid (PLLA) gels.

The second type of gel is polylactic acid gel. After injection, it promotes the formation of a dense subcutaneous collagen matrix, which looks much more natural and firm. Polylactic acid fillers (PLLA, Powerfill) harden in the tissues within 3-5 days, allowing men to engage in sexual activity as early as one week later.
Typically, 20-30 ml of gel is injected in one session, providing an increase of 2-3 cm in circumference.
However, the main drawbacks of polylactic acid gel are its fibrotization (thickening) of subcutaneous tissues, which can cause a slight reduction in penile length during erection. Additionally, reactions to polylactic acid, such as allergic or pro-inflammatory responses, are more common.
Nevertheless, polylactic acid gel is superior to hyaluronic acid as it creates a denser structure, stays in the tissues longer, and dissolves over two to three years.
Working with polylactic acid is more challenging and requires high professional skills from a urologist. The polylactic acid gel must be evenly distributed in the tissue immediately after injection to prevent the formation of lumps and irregularities.

More commonly, mid-range fillers like Hyacorp are used. However, in Dubai and Sharjah, cheaper Chinese (Maxifill) or Korean fillers are often utilized. These are more affordable, priced at 500-1,000 dirhams for 50 ml. However, they lose volume and structural integrity quickly, dissolve faster, and spread unevenly, leading to the same pear-shaped appearance of the penis.

Typically, it is 2,000 dirhams per injection, with the cost of polylactic acid being €50 or 200 dirhams per milliliter. Generally, the volume required is around 20-30 ml, which amounts to approximately 6,000 dirhams for the gel and 2,000 dirhams for the injection, making a total of 8,000 dirhams.
If we are discussing an injection of ActiveGel it is common to administer 40-50 ml of gel in a single session. The total cost of the injections ranges from 14,000 dirhams for 40 ml to 20,000 dirhams for 60 ml. This results in a reduction of up to 5 cm in circumference and significantly increases the penile girth and slightly the length in a flaccid state. This is because 60 ml of gel substantially adds weight, contributing to an elongation effect in a relaxed state.
