Peniflex: A Permanent Solution for Penile Enhancement
If you’re considering a permanent way to increase the size of your penis — particularly its girth and to some extent, its length — the Peniflex silicone implant may be the right solution for you.
Peniflex in Dubai and Europe
Peniflex is a soft, perforated silicone implant available in 5 mm or 7 mm thickness. It is surgically place under the skin of the penis along the corpora cavernosa. The procedure is carefully designed to avoid damage to arteries and nerves, preserving natural sensitivity and erectile function.
Although the implant itself does not enhance sensation directly, many patients report improved sexual experience due to increased penile girth, which enhances vaginal wall contact during intercourse. Importantly, the glans (head of the penis) remains unaffected, as the implant does not interfere with the nerve supply to this area.
Who is a Candidate for Peniflex?
Тo undergo Peniflex implantation, the patient must be circumcised. Circumcision must be performed at least 2 months before the implant surgery.
Additionally, Peniflex implantation can be combined with a ligamentotomy, a procedure that can increase penile length by an additional 2–2.5 cm.
Surgical Approach: Partial Ligamentotomy with Peniflex Implantation
During Peniflex implantation, we typically perform a partial ligamentotomy. This step allows for more precise placement of the implant and contributes to additional penile lengthening. The procedure is safe — it does not affect the angle of erection, nor does it interfere with the blood vessels or nerves responsible for penile function and sensitivity.
A small incision (approximately 4 cm) is made above the base of the penis. In most cases, I use either a Diamond-shaped incision or an inverted V–Y plasty technique. Both approaches offer excellent aesthetic outcomes and can provide additional length to the penis as part of the reconstruction.
Recovery Timeline After Peniflex Implantation
Recovery after Peniflex surgery is relatively quick and straightforward:
• Basic physical activity (walking, working, light movement): after 3–4 days.
• Intensive physical activity (gym, sauna, swimming pool, etc.): after 2 weeks.
• Sexual activity: after 5–6 weeks.
• Oral sex is typically possible after 2 weeks.
Hospital Stay and Surgical Process
Patients are usually asked to arrive at the hospital one day before the surgery for a pre-anesthesia consultation (PAC) and lab tests.
To minimize hospital waiting time, we strongly recommend doing the blood tests one week in advance and sending us the results — this allows the anesthesiologist to approve everything in under 30 minutes. Otherwise, you may need to wait 2–3 hours at the hospital for test results.
The surgery itself takes about 60–90 minutes.
We recommend staying one night in the clinic. The following morning, we perform a dressing change and discharge the patient to their hotel.
For international patients, we advise staying in the city for an additional 3–4 days after discharge. While most patients manage post-op care themselves, we are available for follow-up dressings if needed.
A surgical drain is placed during the operation and should be removed on day 4–5 post-op. After that, the patient is free to travel home.
Post-Operative Complications and Management
The most common and expected post-op occurrence is seroma — fluid accumulation (lymph and blood plasma) around the implant. This is a normal part of healing and helps the formation of a fibrous capsule around the implant.
We usually perform a simple aspiration (puncture) of the seroma around 30–40 days after surgery. This is done under local anesthesia, is completely painless, and takes only 5 minutes. It significantly reduces swelling.
Cases of implant rejection are extremely rare — occurring in less than 2% of patients, similar to other silicone-based implants.
Importance of Proper Implant Selection
Choosing the right implant thickness and shape is crucial. It must fit the patient’s anatomy and also take into account the anatomical compatibility with their partner.
In some cases, we’ve had to adjust the implant size not because of the patient’s dissatisfaction — but because the partner experienced discomfort due to excessive girth.
For patients in stable relationships, we recommend making the decision about Peniflex implantation together with their partner.
Post-Operative Care and Stretching Protocol
After surgery, patients are prescribed:
• Antibiotics: 2 weeks
• Painkillers: 1 week
• Lymphomyosot (anti-edema medication): 2 weeks
We recommend daily bandaging of the penis for the first 2 weeks to reduce swelling.
Starting from week 3, patients should begin gentle manual stretching of the penis, preferably after urination.
From week 4, we recommend switching to a penile extender device. It uses a vacuum system and a leg strap to apply steady traction to the penis.
This post-op traction is crucial: it helps shape a flexible fibrous capsule around the implant that won’t restrict the penis during erection. On the contrary, this stretching technique helps preserve and even increase penile length post-surgery. Thanks to this approach, we often achieve an additional 2 cm of length.

Permanent Penis Girth Enlargement with Peniflex Silicone Implant
The Peniflex thickening penile implant was developed by Dr. Ivan Martinez Aguilar. Features softer silicone (36 mU of elasticity), rounded corner, perforations for lymphatic drainage and tissue integration. It is installed further from the glans, avoiding contact with the pubic bones.
Typically, the Peniflex implantation procedure is performed through a suprapubic approach followed by ligamentotomy to increase the length and thickness of the penis. As a result, the size of the flaccid penis increases by 3-4 cm in both length and circumference, and this increase is maintained during erection.
For example, after surgery, a man's penis, which at rest was 7 cm in length and 9 cm in circumference, becomes 10 cm in length and 12-13 cm in circumference. When it erects, the dimensions also increase from 14 cm in length and 12 cm in circumference to 16 cm in length and 17 cm in circumference.
Peniflex is primarily suitable for those who want to increase the size of their erect penis and improve their sex life. Soft silicone does not reduce sensitivity, but, on the contrary, improves contact with the partner’s vagina. The implant is placed at a distance of 1.5 cm from the head of the penis, ensuring the need to use a hanger, stretcher or extender for lengthening in the postoperative period and the formation of a stretchable capsule that will not limit the size of the penis during erection. The antifibrotic properties of soft silicone and the lack of emphasis on the pubic bones also reduce the risk of developing a fibrous capsule and reduce the invasiveness of the procedure.
For the same reason, the probability of developing various complications, primarily implant protrusion, is significantly less and is only 3.2% over an observation period of 8 years.
Penuma (Himplant) in Dubai and Europe
Dr. Elist from California has developed an implant with a right-angle end and low-elasticity silicone, which is installed under the head, adjacent to the pubic bones. This results in an increase in the flaccid g penis by 3-4 cm in circumference and length due to partial ligamentotomy and constant traction of the implant.
Over time, a fibrous capsule forms around the implant, which maintains the increased size of the flaccid penis, but interferes with maximum erection of the penis. So, for example, if a person with a length and circumference of the penis in a flaccid state of 7 cm and 9 cm, and in an erection with a length of 14 cm and a circumference of 12 cm, a Penuma implant was installed, then after surgery its dimensions become 10 cm in length and 13 cm in circumference and 13 cm in length at flaccid and 17 cm in circumference when erect.
In other words, the length of the erect penis may decrease, which is a concern for some patients.
The Penuma penile enhancement is designed for men who primarily want to increase the size of the flaccid penis and solve problems with visibility when undressing.
The second “disadvantage” of the Penuma procedure is frequent protrusion and prolapse near the skin of the head of the penis, caused by constant pressure of the implant in the area of the pubic bones. This can lead to skin defects and infections, forcing the removal of a significant proportion of implants (20- 22%). This was presented at the joint congress of SMSNA and ISSM (North American and International
Society of Sexual Medicine) in Miami, USA, in October 2022. Many American andrological surgeons have already declared their high volume experience in explanting (removing) the Penuma implant.
